Keeping the assessment criteria in mind (uniqueness and originality; development; operational benefits; adoption and/or commercialization; broader use, impact or benefits), provide thorough and complete answers for the following questions. Pictures, drawings and letters of support may strengthen your application/nomination. Please use quantitative information when available (# of acres, # of employees, financial benefits of innovation, etc.)

1. (development, uniqueness and originality)

Describe the evolution of your innovation, including if you developed this innovation yourself, modified existing technology or adopted it from somewhere else. Highlight what makes your innovation unique and original. Please outline the process used in developing, modifying or adapting your innovation and what stage of development it is in (concept, design, prototype, development, in-use)?

2. (operational benefits)

What operational benefits does the innovation provide (examples could include increases income, increases efficiency, improves health and safety, reduction in energy, increased yields, improved product quality)?

3. (broader use/adoption and/or commercialization)

What is the potential broader use for the innovation, beyond your farm? Has the innovation actually been adopted by other producers? If so, provide examples. Has the innovation been commercialized? If so, how? If not, outline how that might be accomplished.

4. (impact and benefits)

How will your innovation have a positive impact for the Ontario agri-food sector and what are the economic and/or social benefits as a result of the innovation (benefits to the community, the environment, job creation, economic impact)?

All the following criteria must be met to be considered eligible (please check boxes): Applicants must:

be a farmer / primary producer, group of farmers, combination of agri-food businesses, or an agri-food related organization (a farmer must be one of the applicants of any group)
be a resident of Ontario
be a Canadian citizen or permanent resident of Canada.

The information you supply on the application form is being collected by the Ontario Ministry of Agriculture, Food and Rural Affairs, Economic Development Division and used and disclosed for the purpose of administering and promoting the Premier's Award for Agri-Food Innovation Excellence. This includes processing your application, confirming your program eligibility, and contacting you in the event that you are a finalist and/or winner. Your information may also be shared with external regional review panels. In the event that you are a finalist, your name, contact information and photographs may also be disclosed in print and/or electronic form to promote the objectives of the Agri-Food Innovation Excellence award as set out in the 2009 Application / Nomination Guidebook. Recipients of this award will also be recognized at award events in 2010.
